Field Trip Release of Liability

Since many Cañon City Geology Club's field trips during 2023/2024 will be joint field trips with the Pueblo Rock Hounds and Columbine Gem and Mineral Society, a combined Field Trip Release of Liability Form has been devised to cover all three clubs.  The first page of the form is shown below.  Click on the picture to download all pages of the form, fill it out, and turn it in at a member meeting, or give the from to the field trip leader.  (Only required once; rolls over year to year.)
